To Submit the Faculty Mentor Recommendation Form:

1. Enter the Student's X.500 ID in the box below. (Example: xxxx1234 not xxxx1234@umn.edu)

2. Enter the Faculty Mentor's X.500 ID in the box below. (Example: marshak not marshak@umn.edu)

3. Use the "Browse" button to select the appropriate file.

4. Click on the "Submit" Button.
